Menu bar icon to add new database entry?

The "+" (Add) button in the menu bar seems to add a new group, which happens much less frequently as adding a new database entry. However there seems to be no button to add a new database entry. The customize toolbar also does not give an option to add this "New Entry" function button in the toolbar...
